What to bring to your first visit
To get you registered with FMP (if needed) and billed correctly, bring the following to your first appointment:
- Government-issued photo ID (U.S. driver's license, passport, or DR residency)
- Your DD-214 (discharge paperwork)
- Your VA rating decision letter, if you have it
- Your FMP ID number, if already registered
- A list of current medications
- Any recent lab results or imaging from U.S. providers (optional but helpful)
